NO-SMOKING ADDENDUM to the

Comfort Inn guest room rental agreement

 

This mandatory No-smoking Addendum is entered between the Comfort Inn of Cross Lanes WV and                                                   our Guest and/or credit/debit card holder renting room number (s)                                         on this           day of__________20        . This addendum is an integral part of the Hotel room rental agreement entered between the Comfort Inn and the guest and the credit/debit card holder as it relates to being a guest, visitor and/or financially responsible party of the Comfort Inn, 102 Racer Dr, Cross Lanes, WV

 

1.         Definition of smoking: smoking includes but not limited to inhaling, exhaling, breathing, carrying, lighting, or possession of any lighted cigarette, cigar, and pipe, other products containing any amount of tobacco, smoke or any other lighted products, electronic cigarettes, and/or illegal drugs or items.

 

2.         No smoking rule: Guest, visitors of guest and/or credit/debit card holder agrees and acknowledges that Smoking as defined is prohibited in the entire hotel including all rooms, all public and non public areas inside the Comfort Inn and all other areas within 20 ft of the hotel and that the guest and/or visitors of any guest shall not smoke as defined above. The registered Guest and Credit/ Debit card holder are also responsible for all their visitors and their guests of the room that was rented and also when in public areas of the hotel.

 

3. Fine:  Guest and/or Credit/Debit card holder agrees that that the fine for violating this agreement is $1000.00 and this fine is issued at the sole discretion of the hotel management and can not be disputed.

 

By signing below I acknowledge have read this agreement and agree with it.
